Log, certification and parental consent
The DSD X 152 practice log is supplied for convenience. It records dates, locations, weather, day/night time and initials, with a final signature line. Keep it accurate as you practise.
The Parent-Teen Driving Guide helps families plan practice. At licensing, the parent or guardian certifies the completed hours. Under the GDL instructions, they accompany the teen to provide written consent, or complete and notarize the Affidavit/Consent for Minor to Drive. Do not assume that signing an app printout completes both steps.
A filled-in example: a useful supporting record
Illustrative entries show how a record can preserve the day/night split. Put the responsible adult's initials beside real entries; do not copy this example as completed practice.

Mistakes that confuse the documents
- Confusing the convenience log with parental consent to licensing.
- Reaching 50 hours without 10 at night, or overlooking the 9-month permit period.
- Assuming every family needs a notarized affidavit even when the parent attends and supplies consent.
- Counting instructor time as part of the separate supervised practice shown on DSD X 152.
