No, and it says so on every page it prints. It is a dated supporting record of your drives. Some states accept a log like this; others require their own form — each state guide tells you which, with a link to the official document.
No. There is nothing to sign up for. Your data lives on your iPhone and, if you enable iCloud, in your own private iCloud storage. Family members can access only the private shares you create.
